The presentation will include an overview of dual-diagnosis treatment planning for continuing care after residential treatment. 449 Recovery’s treatment model will be presented, including facets of conjunctive care with sober living and other community services.  “Beyond the 90” specifically focuses on how to keep clients engaged in their treatment and invested in long-term well-being after a full continuum of care is completed, including Alumni programming, connection with community resources, and sustaining the therapeutic alliance indefinitely.

Differentiate between the 3 levels of outpatient dual-diagnosis levels of care, including Partial Hospitalization (PHP), Intensive Outpatient (IOP) and Outpatient (OP), within the context of a continuum of care treatment plan.

Identify and define 3 facets of an Alumni program that increase a person’s potential for engagement in care beyond program completion.

Identify 3 community based resources for clients that complete a 90-day program with a dual-diagnosis treatment plan in an outpatient setting.
